Essential Notary Forms
Tips to help improve Business Image
A Special Thank You to all who
have contributed forms to this page.
Notary Disclaimer:
Print a bunch of these. Insert this Disclaimer into your loan
package the minute it comes off the printer. This way you won't forget when you
get to the appointment. This disclosure has a dual purpose. 1) The borrower will
sign that they have been made aware (by you) that you cannot advise them. 2) The
borrower signs that you did administer the oath required by law. Now do you
understand this oath and what it is for? This oath is administered to swear
truthfulness to the jurats in the loan package. A jurat has the wording 'Sworn
and subscribed', right? The oath is them swearing the truthfulness of this
document with a jurat on it. If you DO NOT administer this oath, it can effect
the validity of the notarization.
Refusal to Sign:
You are at a signing and the borrower decides not to sign. For
your protection, you may want to get a copy of the 'Refusal to Sign' signed by
the borrower explaining why he is refusing to sign.
All-Purpose Acknowledgement
This acknowledgement may
not meet your states minimum requirements
Minor Medical Release Form
This might be used with
a daycare provider or relative in the event a child is injured or becomes
severely ill while in their care.
Child Travel Permission Letter
This form might be used
if a child were being taken out of the country by only one parent. This is a
required document to travel with a child without both parents or for another
relative to take them out of the country.
Collection Letter:
This is a wonderful collection letter I got from a lady on a
forum. If I remembered her name I would post it here. Since I can't I will
simply thank her here for a wonderful collection letter that packs a punch.
W-9:
If you have a version of Adobe (Pro) that will allow you to
fill it in, Great! If not, simply print it out, fill it out and sign it and scan
or have it scanned to disc to get it back in your system. You should be able to
find an office supply store that will do this for you.
Fax Coversheet:
Do a Save As to your computer, fill in your information and save. Now
each time you need to send a fax, open the file, fill in the contact info and
message, print (and DON'T save) then you will always keep your template ready
for entering.
Shipping Label:
Use 8164 Avery form (8-1/2" x 11”) forms bought at any office supply
store. You don’t have to print an entire page. Choose which label squares to
fill in depending on where you want it to print on the label page. Example, if
you have already previously used the first one at top left then enter your info
into the third and fourth square down on left in the word document and then it
will print on the second label down on the left. Leave the rest of the squares
blank. The first box at top left is for your address the next box down the same
size is to input where you are sending it.
Business Card:
Printing your own business cards has it's advantages. If you
start out as a Uncertified Signing Agent and in your excitement you order 1000
business cards and a month later you get certified your business cards will be
outdated. You can purchase a packet of 8-1/2" x 11" sheets of perforated
business cards at any office supply store. Using the template on this page, fill
in the info you want on one of the squares. When you have it just the way you
want it, highlight with your cursor, right click, copy and paste to each square.
Additional Tips:
While scanning the W-9, you may also want to scan in your
Notary Certificate, E&O Insurance, Bond and any other items you have needed to
send in to sign up with a company. This will save you having to make a copy each
time and faxing it in. Simply attach them all in an email and email them. Saves
on paper and long distance if you don't have a toll free fax number for them.
